These forms express interest; they do not register someone for a program or confirm a volunteer role.
Before you download
Participant form privacy notice
For minors, a parent or guardian should be the primary contact. Do not provide a minor’s full name, birth date, personal contact details, home address, school, photo, or social media information. Participants age 18 may provide their own contact information. Additional permissions may be requested for a specific activity.
The form asks for an adult contact’s name and email, optional phone and general location, and interests and activity preferences. Imagine You-CD will use the information you choose to return to understand your interests and follow up about relevant opportunities. Updates are optional on the PDF.
Completed forms are returned by ordinary email to info@imagineyou-cd.com. Email attachments are not an encrypted upload; your mail service and the recipient mailbox process the message. Include only the information requested on the form and do not add sensitive details about a minor.

Three simple steps
How to return your form
- Download the appropriate PDF above.
- Complete and save it in a PDF app. Provide the contact details and interests requested on your chosen form. Keep a copy for yourself.
- Email the saved PDF as an attachment to info@imagineyou-cd.com. Check that the filled PDF is actually attached before sending.
The email link opens your mail app if one is configured. It does not attach the completed PDF automatically; you must attach the saved copy yourself.
